Financial Aid Forms
IMPORTANT: Forms with an asterisk (*) must be printed out, completed in black or blue ink, and the original turned in to the Financial Aid Office. Completed forms will be scanned and processed. Please print clearly and follow the directions for completing the form to insure accuracy and to avoid a delay in processing your information.
Application Process
FAFSA - Free Application for Federal Student Aid. Determine the correct year to complete based on your enrollment:
2009-2010 (for Summer 2009; Fall 2009; Winter 2010)
First Time Applicants:
- Print and complete a FAFSA on the Web Worksheet
- The process of obtaining a Personal Identification Number (PIN) is easy at www.pin.ed.gov. The PIN is your identifier to let you access personal information in various U.S. Department of Education systems. You must have a PIN to file an online Renewal FAFSA or to make online corrections to your FAFSA. If this is your first time filling out the FAFSA online, and you haven't applied for a (PIN), you should do so now.
- When the FAFSA asks for the the School Code,Franklin's Federal School Code is 003046.
- Sign your application electronically using your Federal Student Aid Personal Identification Number (PIN).
- If you are a dependent student, one of your parents will also have to sign your application. Your parent can apply for a PIN and sign electronically also.

Ohio Student Choice Grant Application (complete only if you are not completing the FAFSA)*
This State of Ohio grant is not need-based, and is used for tuition only. To be eligible for the Ohio Student Choice Grant, you must meet the following requirements:
- Full-time student status (12 semester hours)
- Seeking first bachelor's degree
- Ohio residency of 1 year required
- No full-time, post-secondary attendance prior to July 1, 1984
Privacy Release* - Due to Federal regulations, we must have signed authorization if you would like our office to release information about your account to someone other than yourself (spouse/parent, if not a dependent student.)
Other Forms that may be requested:
Selective Service Registration * - Complete and follow the instructions on this form if you received a notice that you need to provide proof that you are registered with selective service or documentation that you are exempt from registering.
Verification Process (Complete only if notified by Financial Aid)
The Department of Education randomly selects a portion of aid applicants for a process called Verification. If selected for verification, our office will send you a letter informing you of which verification form you need to complete and which copies of the IRS tax return you will need to submit. The Verification Worksheet consists of much of the information that was requested on your FAFSA form.

If you (or your spouse or parent) do not keep a copy of the Federal IRS Tax Return our office is requesting, you can obtain a 4506-T transcript by calling 1.800.829.1040 or by completing and mailing Form 4506-T. Checkmark #6 b for tax filer and #7 for verification of being a non-tax filers.
The IRS will mail the transcript(s) directly to you within two weeks of your request being received. Note: after receiving your transcript from the IRS, sign and date the transcript and submit it to the Financial Aid Office.
Loan Process
- Master Promissory Note (MPN)
The binding legal document you sign when you get a student loan. It lists the conditions under which you’re borrowing and the terms under which you agree to pay back the loan. It will include information on how interest is calculated and what the deferment and cancellation provisions are. It’s very important to read and save this document because you’ll need to refer to it later when you begin repaying your loan. If you have borrowed student loans at Franklin, you do not need to complete another MPN. The MPN may remain valid at Franklin for ten years. - Student Loan Entrance Counseling
Federal requirement for all first time student loan borrowers. It explains your rights and responsibilities as a borrower of federal student loans. - Student Loan Exit Counseling
Exit Counseling is a federal requirement for all federal student loan borrowers who are graduating (bachelor's or master's degree) or students who withdraw or drop below half-time (6 semester hours) for six months. A hold will be placed on transcript requests until the federally required Counseling has been completed.

This is an internal application that is required each award year before Franklin will certify enrollment at the Department of Veterans Affairs. If you are a new or transfer student, or are changing your program of study, please contact Franklin's Veterans Coordinator for additional requirements.
