Tax Return Information for 2008 (Federal 1040, 1040-EZ, 1040A, etc)
Need a copy of your Federal IRS Tax Return? Obtain a 4506-T transcript by completing and mailing Form 4506-T. (Check number 6b for tax filer and number 7 for verification of being a non-tax filer.) The IRS will mail the transcript(s) directly to you within two weeks of your request being received. After receiving your transcript from the IRS, sign and date the transcript and submit it to the Financial Aid office.
Privacy Release
Due to Federal regulations, we must have signed authorization if you would like our office to release information about your account to someone other than yourself (spouse/parent, if not a dependent student.)

*Print out, complete legibly in blue or black ink, and submit to the Financial Aid Office for Processing.
Community College Alliance Students Only:
All students participating in the Community College Alliance Program are required to submit a new Student Contract each financial aid award year. Follow the completion and submission instructions printed on the Student Contract
